We are a Catholic school community based in Beach Park, Illinois serving families from the surrounding area. We welcome all applicants for enrollment in PREK, Kindergarten, Elementary and Middle School.
​
Our Lady of Humility offers an all-encompassing approach to engage students spiritually, physically, emotionally and academically. Catholic tradition is woven throughout student life. Our children develop into life-long learners, form everlasting friendships, serve God and care for others.
Enrollment Process
**ENROLLMENT WILL OPEN on January 26, 2025
for 2025-2026 School Year**
Our Lady of Humility welcomes all applicants. Students must be three years old by September 1 for preschool, and five years old by September 1 to enter
kindergarten their starting year.

New OLH Students or Adding additional students to a Current OLH family:
*Incoming student(s) and parent(s) will complete an interview with Mrs. Valdes (4th - 8th).
*Incoming students will complete a tour (PK - 3rd) or shadow day (4th - 8th).

Set up FACTS account for tuition management *If you are a current OLH family adding a student, your student will be added to FACTS once the NEW STUDENT APPLICATION and TUITION CONTRACT have been completed.

Additional Forms & Documents Needed for Registration:

Annual physical - New students, students participating in sports, and students entering PK, K, and 6th
-
Students entering preschool, kindergarten, and 6th grade are required to have an Illinois state physical and must have a diabetic screening (K & 6th) and lead screening (PK & K)

All immunizations need to be up-to-date. Students entering 6th grade need a T-dap booster and Meningococcal Vaccine.

Medical and Emergency Notification Information- Required for all students

Medication Authorization/Physician's Order - ONLY if your student requires a medical action plan

Dental - students in K, 2nd, and 6th grade

Eye Exam - kindergarten and anyone entering school for the first time (preschool excluded)

Complete Bus Form online - *only if student(s) who will be riding the bus with Zion (monthly fee/billed separately) or Beach Park

Complete EDP Form online - only for students that will be utilizing before/after school care program
**Please note that the Illinois Exempt Date is October 15th**
