Our Lady of Humility School

• Any student entering the school from outside of Illinois must have a new physical. Students entering an Illinois school for the first time must have an eye exam (Pre-K excluded).

• Students entering Preschool, Kindergarten, and 6th grade are required to have a physical and must have a diabetic screening (Kindergarten and 6th) and lead screening (Pre-K and Kindergarten).  All immunizations must be up-to-date.

• Students entering 6th grade need a T-dap booster and Meningococcal Vaccine. 

• Students entering Kindergarten, 2nd and 6th are required to have a dental exam.

• Only Kindergarten and anyone who is entering school for the first time (pre-school excluded) must have an eye exam

*ALL STUDENTS PARTICIPATING IN ATHLETICS ARE REQUIRED TO HAVE AN ATHLETICS PHYSICAL COMPLETED WITHIN THE PREVIOUS 12 MONTHS OF PARTICIPATION.*
